Please note that filling this form does not guarantee getting an appointment. If this a dental emergency, it is best to call the office directly to book with us and not sent in an appointment request form. We are open Monday through Thursday, office opening at 9 AM.

Since we are a private practice, we book our patients 6 month cleaning at their cleaning appointment, so it will not be uncommon to be booked out a few months in advanced.

We always strive to accommodate our new and returning patients as much as possible. We highly recommend booking the appointment at our next available, and when a cancellation comes we will go through our cancellation list and get patients in sooner (new and returning).
