Uptown Dental Associates
Thank you for selecting us as your dental health care provider. Our primary goal is that you receive the best treatments needed to restore and maintain your dental health. If you have questions or concerns about our cancellation policy, please do not hesitate to ask one of our office administrators.
We make every effort to be on time for our patients and ask that you extend the same courtesy to us. We will call, text or email to confirm your appointment 1-2 days beforehand. Please make sure that your contact information is correct.
If you need to change or cancel an appointment, we require a two-business day advance notice. Inability on our part to confirm your appointment does not absolve you of your responsibility to notify us. This courtesy on your part will make it possible for us to offer the appointment to another patient.
Applicable fees apply for not honoring this policy, your failed appointment or last-minute cancellation will incur a fee of:
$50 for routine visits, such as but not limited to, exams and hygiene appointments.
$100 for minor procedures, such as but not limited to, fillings, extractions, and deep cleaning appointments.
$250 for major procedures, such as but not limited to, crowns, implants, bridges, and sedation appointments.
We understand that situations may arise that would make it impossible for you to give full notice. If extenuating circumstances prevent you from keeping your appointment, please explain them to a front office team member. Each incident will be given consideration based upon your appointment history.
Thank you for putting your faith and trust in us at Uptown Dental Associates.
